Troop 394 - Forms

Click on the name of the document you wish to open, then click “Save” when the download box appears. Choose a location to save the file in order to keep a copy for yourself.
Note: Files exist in three formats – Word Documents (DOC), Portable Documents (PDF), and Fillable Portable Documents (FPDF). Word documents will download directly to your computer when you click on them, and you can fill them out on the computer and edit the text within the document. For portable documents, you cannot edit the text, but you can print and fill it out. Fillable portable documents, however, allow you to fill out the form on the computer.

For the Scouts:
  1. The Basics of Scouting (PDF) – How to do knots, lashings, first aid, and much more.
  2. Campout Suggestion (PDF) – You can now suggest a brand-new campout to the SPL and the scoutmaster, but you have to do a little bit of research on it first. Fill out and submit to the SPL or ASPL.
  3. Leadership Application (PDF)- This form must be filled out before applying for a new job at the start of every term. You must state your rank, current and past positions, your desired position, and why you are the best suited for it. You must also obtain parent signatures.
For the Minutes Scribe:
  • PLC Minutes (PDF) – A form to be taken to PLCs and filled out by the scribe. It contains areas for job reports, assignments (to be given out by the SPL), New Business, and Old Business. Enough should be printed out for the Adult Leaders and the Leadership present, including a copy for the scribe himself. A scribe should keep the minutes of all PLC’s until the end of his term, as they frequently need to be read out at the beginning and end of the next PLC. The minutes, upon completion of the PLC, are to be typed and sent to the leadership and the Scoutmaster.

For the Quartermaster:

  1. Trailer Status (PDF) – An inventory to be filled out by the quartermaster every time a trailer cleanup occurs. The quartermaster is to coordinate, plan, and execute the event; and can use this form as a guide to the materials in the trailer. Should stay in the possession of the quartermaster until the next trailer cleanup. Can be used as a list for current items, items to be thrown out, and items to be newly purchased.
  2. Trailer Checklist (PDF) – A checklist of objects that should be taken to every campout. Should be retained in the possession of the quartermaster. Should be brought to all trailer pickups prior to campouts, and the quartermaster should ensure that all items on the list are taken. List may be adjusted for specific campouts. Upon the conclusion of the campout, the list should be run through to ensure that all items have been brought back.
For the Patrol Leaders:
  1. Camping Essentials Checklist (PDF) – A list of all campout essentials you should have with you at all times on an outing; includes a checklist for all items needed in the patrol box
  2. Duty Roster (DOC) – A form that must be filled out consisting of the task assignments for each campout, including cooks, kitchen patrol, and setup/cleanup. This file is in Word format, so it can be typed and printed out for final approval
  3. Campout Menu Planner (DOC) – A form that must be filled out consisting of the meal plans from Friday night to Sunday afternoon for each campout. This file is in Word format, so it can be typed and printed out for final approval. Also has a mini-Duty Roster for quick access.
  4. Fun / Service Projects (PDF) – A write-up that must be filled out and turned in to the SPL after completing any patrol events such as a fun or service project
  5. Tour Permit (FPDF) – A form that you must fill out and get council approval for before you can go on a service project or any other such event.

    • The stamped, signed form will be put into the mail. The troop has to have the  original, physical copy of the permit. BSA is not permitted to e mail or fax the form back. This process can take  3 to 5 days
  6. National Honor Patol Award Tracker (DOC) – A form that summarizes the accomplishments necessary to achieve the National Honor Patrol Award. Must be completed over a span of three months and turned into the Scoutmaster before the deadline for credit

For the Senior Patrol Leaders:

  1. Camp Plan (PDF)- A plan to be filled out for all campouts, including objectives, expectations, a gear list, and a schedule for the weekend.
  2. Permission Slip (DOC) – A standard permission slip to be filled out by all going on that event in order to obtain permission to go
  3. Jobs & Duties (PDF) – A list of jobs and duties for each and every position in the Troop. By the end of one’s term, all of the required fields must be accomplished to appropriate standards or job credits will not be received.

For the Parents:
  1. Annual Health & Medical Record ABC (FPDF) – Mandatory medical forms for all scouts going on a campout. Should be filled out annually and given to the outings coordinator.
  2. Expense Form (PDF) – Form must be filled out and given to the treasurer along with a receipt for reimbursement
  3. Firearms Release (PDF) – Gives permission for a boy to shoot arrows, rifles, and shotguns. To be filled out whenever necessary.
  4. Liability Waiver (PDF) – Form stating that the Boy Scouts of America will not be held responsible for any injuries that can happen to a boy. To be filled out whenever necessary.
  5. Medical Treatment (PDF) – Form allowing the Boy Scouts of America to treat a boy in any case of injury or emergency. To be filled out whenever necessary.
  6. Talent Release (PDF) – Allows the Boy Scouts of America to use photos or videos of a boy for educational purposes. To be filled out whenever necessary.
  7. Scout Record (PDF) – Template for the Advancement Committee to keep a record of each Scout’s progress on his achievements and rank advancements

For *Potential* Eagle Scouts:
  1. Life to Eagle Process (PDF) – A presentation detailing the steps that you need to take from Life to Eagle
  2. Eagle Project
    1. Project Criteria (PDF) – Lists the conditions under which an Eagle Project will be approved
    2. Project FAQ (PDF) – Addresses some common inquiries on the Eagle Project
    3. Project Planning Guide (PDF) – A guide to help you get started on your leadership service project
    4. EP Parents Guide (PDF) – A list of things to help parents guide their son through the project
    5. Beneficiary Letter (PDF) – A letter to give to the benefiting organization to tell them what an Eagle Project is about.
    6. 501-C3 Letter (PDF) – A letter to give to potential contributors identifying the BSA as a tax-exempt group
    7. Eagle Project Workbook(FPDF) – To be filled out and turned in to Scoutmaster in order to complete and execute Eagle Service Project; MUST complete (up to page 9) this in order to get District Approval

    9. Local Tour Permit (FPDF) – To be obtained from the District to allow the scouts to work in the Eagle Project
